Post-Service Client Survey

Purpose

The purpose of this questionnaire is to gather consistent, client-centered feedback about doula support after services have been completed. This tool is designed to reflect the core values of doula care while honoring the diversity of practice styles across Alabama. This is not a performance review, complaint form, or rating system. It is a reflective tool that centers the client’s lived experience and helps elevate the standard of doula care statewide.

Why This Matters

Doula work is relational, nuanced, and deeply personal. Yet, without shared language or consistent evaluation tools, it can be difficult to:

  • Understand how clients experience doula support

  • Identify strengths and growth areas across the profession

  • Demonstrate the value of doula care to communities, partners, and stakeholders

  • Protect the integrity and scope of doula work

This questionnaire creates a common framework that respects autonomy, emotional safety, informed decision-making, and ethical support without imposing a single model of care.

How the Questionnaire Is Intended to Be Used

  • Sent to clients after doula services have concluded

  • Optional for doulas to distribute, but encouraged as a best practice

  • Aggregated data may be used by the Alliance in the future only with transparency and consent
